Correctional Linkage to Care (CLTC) – Virtual capacity-building sessions for providers

In the summer of 2021, IDTA@JSI’s project worked with the Massachusetts Department of Public Health (MDPH) to host three virtual capacity-building sessions on Correctional Linkage to Care (CLTC) services. Participants in the CLTC capacity-building series included providers from agencies receiving funding from the MDPH Bureau of Infectious Disease and Laboratory Sciences (BIDLS) to provide CLTC services. Session activities included agency panel interviews and group discussions. Video recordings and slide decks for each session are available to view and download below.

CLTC Session 1: CLTC Referral and Enrollment

Tuesday, July 13, 2021, 1:00-3:00pm

This session included an in-depth review and discussion of Correctional Linkage-to-Care (CLTC) referral and enrollment, along with a panel presentation with Cambridge Health Alliance and Health Imperatives staff who shared their processes for testing and referring clients.

This session focused on increasing participants’ ability to:

  • Describe the basics of CLTC services and expected outcomes.
  • Define the referral and enrollment components of CLTC and expected outcomes.
  • Identify corresponding indicators for referral and enrollment components.
  • Identify at least one model for referral and enrollment into CLTC.

CLTC Session 2: CLTC Pre-Release and Post-Release Activities

Tuesday, August 3, 2021, 1:00-3:00pm

This session included an in-depth review and discussion of the pre-release and post-release activities associated with Correctional Linkage-to-Care (CLTC), along with a panel presentation with Boston Health Care for the Homeless, Baystate Medical Center, and Seven Hills Behavioral Health staff who shared the services they provide in correctional facilities and for post-release coordination.

This session aimed to increase participants’ ability to: 

  • Describe the basics of pre-release and post-release activities and expected outcomes.
  • Identify at least one model for pre-release and post-release engagement with clients.
  • Identify at least one approach to addressing common challenges related to pre-release and post-release engagement.
  • Identify corresponding indicators for pre-release and post-release.

CLTC Session 3: CLTC Reporting

Tuesday, August 24, 2021, 1:00-3:00pm

This session included an in-depth review and discussion of Correctional Linkage-to-Care (CLTC) reporting, including an overview of the service definitions, and CAREWare reports, along with a panel presentation with Baystate Medical Center staff who shared their methods for tracking and reviewing CLTC services.

This session aimed to increase participants’ ability to:

  • Describe CLTC reporting requirements.
  • Identify program indicators and how they can be used for evaluation and quality improvement.
  • Discuss ways to apply data to inform CLTC service delivery with other staff in their agency.
